Principal Care Management Care Coordinator

About the Role

MTC Care is the nation's leading telemedicine provider of behavioral health care to skilled nursing, assisted living, and independent living facilities. We are seeking an experienced full-time Principal Care Management (PCM) Care Coordinator for a remote position.

Essential Functions:

• Assist clinicians with gathering medical information for patients in-between visits utilizing different electronic health record systems.
• Report medical concerns monthly, including falls, medication changes, behavioral concerns, and recent hospitalizations.
• Complete PCM assignments for approximately 15 patients per day, ensuring all interventions are finished each month.
• Report issues directly to the ancillary program manager or Chief Clinical Officer.
• Support employee training and provide assistance to new staff members via shadowing.

Knowledge, Skills, and Abilities:

• High level of organizational skills and proven follow-up ability.
• Basic knowledge of medical terminology and strong attention to detail.
• Excellent written and verbal communication skills.
• Proficiency in healthcare technologies, including Electronic Health Record (EHR) systems, Microsoft Outlook, and Microsoft Excel.

Education and Licensing:

• Minimum High School Diploma required.
• Must have a quiet, distraction-free home workspace with reliable internet access.
