Pharmacy Liaison

About the Role

The Pharmacy Liaison provides integrated off-site support to patients, pharmacists, and providers, ensuring a full-service approach to total quality patient care. The role focuses on delivering a full continuum of medication adherence support and encouraging the use of hospital partner pharmacies.

Job duties include:

• Providing custom medication management services and patient onboarding.
• Handling prior authorizations, financial assistance, and refill/renewal support.
• Educating prospective patients on hospital pharmacy benefits and services.
• Resolving insurance-related issues and completing benefit investigations.
• Documenting patient information accurately within the hospital EMR and Therigy systems.
