Imagine Pediatrics is a tech-enabled, pediatrician-led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual-first and in-home medical, behavioral, and social care, working alongside families and providers to break down barriers to quality care.

The Care Team Assistant Manager, Transitions of Care, will oversee the Care Team Assistants and Care Coordinator roles responsible for supporting our Transition of Care team. This is a largely administrative leadership role overseeing a non-clinical support team that works closely with Transition of Care Managers.

Key responsibilities include:

• Overseeing Care Team Assistants and Care Coordinators.
• Managing the downloading and scrubbing of data.
• Ensuring accurate chart preparation for providers.
• Tracking caseloads and requesting medical records.
• Providing general administrative support to multiple teams.
