Veteran Pathways & Proactive Care is offering a unique opportunity for an experienced General Practitioner to escape the high-volume grind and transition into a more meaningful, structured clinical model. This role is specifically designed for a GP who values thorough, evidence-based care and clinical leadership, focusing on chronic disease management and structured multidisciplinary case conferencing for veterans and patients with complex, multi-system health needs. By splitting your week between a state-of-the-art practice in Ipswich and the comfort of your own home office, you can achieve a genuine work-life balance while delivering high-impact healthcare to a community that truly needs it.

As a key member of our Chronic Care Team, you will move away from standard locum work and into a role where you provide genuine clinical leadership. You will work alongside a dedicated team of allied health professionals and specialists to translate complex clinical findings into coordinated, evidence-based care plans. Our model prioritizes patient outcomes and thoroughness over throughput, ensuring you have the time and resources necessary to manage patients with chronic pain, mental health comorbidities, and musculoskeletal injuries. Ipswich is a DPA-eligible location, and we warmly welcome applications from International Medical Graduates eligible for a 19AB exemption.

The working pattern is designed for sustainability and flexibility. You will spend one 8-hour day (typically Tuesday) at our well-established Ipswich partner practice, which serves as a natural multidisciplinary hub for chronic care. Your second 8-hour day (typically Thursday) is conducted entirely via telehealth from your home office, focusing on structured case conferencing. While these days are the standard, we offer flexibility to arrange the schedule around your existing professional or personal commitments.

Key Responsibilities

In this role, you will lead the clinical direction for patients within a multidisciplinary framework, ensuring high-quality continuity of care through the following activities:

  • Conducting comprehensive GP consultations for acute, chronic, and complex conditions within the Chronic Care Team framework.
  • Delivering preventative health assessments, including cardiovascular risk profiling, metabolic health reviews, and mental health screenings.
  • Managing Chronic Disease Management (CDM) reviews and care planning, specifically focusing on GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
  • Leading scheduled telehealth case conferences, integrating input from pathology, imaging, and allied health providers into cohesive patient care plans.
  • Overseeing pathology and imaging investigations, including medication management and referrals to relevant specialists and community services.
  • Ensuring clinical safety through the identification and actioning of clear prioritization and escalation pathways.
  • Collaborating with the Department of Veterans' Affairs (DVA) and utilizing the Coordinated Veterans Care (CVC) Program to support the veteran community.
Qualifications

To be successful in this clinical leadership role, candidates must possess the following qualifications and experience:

  • Medical degree (MBBS or equivalent) with current, unrestricted registration with the Medical Board of Australia (AHPRA).
  • Fellowship with either the RACGP or ACRRM (VR GP status).
  • Eligibility for a 19AB exemption for those applying in the DPA location (IMGs are encouraged to apply).
  • Demonstrated experience in chronic disease management and the coordination of complex patient care.
  • High level of competence in clinical prescribing, referral management, and the interpretation of investigations.
  • Strong communication skills with a proven ability to collaborate effectively within a multidisciplinary team environment.
  • A dedicated home-office setup with a reliable internet connection for the telehealth component of the role.
  • Prior experience with DVA billing or the CVC Program is highly regarded but not essential.
