About the Role

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a meticulous and proactive Documentation Management Specialist to join our award-winning team. In this vital role, you will serve as a bridge between our clinical operations and referral partners, ensuring that patient care documentation is accurate, compliant, and processed with urgency to support our mission of delivering compassionate healthcare at home.

Key Responsibilities

As a Documentation Management Specialist, you will be responsible for the end-to-end management of medical orders and compliance documentation. Your primary focus will be ensuring that all physician signatures and Face-to-Face (F2F) encounters are captured within regulatory timeframes to prevent delays in patient care.

  • Collaborate closely with Clinical Liaisons (CCs) to visit referral partner offices, fostering strong professional relationships and understanding specific account preferences for document delivery.
  • Conduct regular field travel to physician offices and medical facilities to obtain physical signatures on current and outstanding medical orders.
  • Monitor and audit physician profile data within the Electronic Medical Record (EMR) system to ensure accuracy, researching and correcting discrepancies as they arise.
  • Maintain rigorous documentation logs of all follow-up activities, tracking progress by date and action to ensure a clear audit trail of efforts made to secure missing paperwork.
  • Analyze weekly reports to identify outstanding F2F encounters and orders, working strategically with the regional team to ensure completion within 14 days of patient admission.
  • Facilitate the escalation of delayed orders by coordinating between the home office, referral partners, and clinical staff to resolve bottlenecks.
  • Prepare and organize weekly order batches for field staff and participate in departmental meetings to provide status updates on high-priority or aging accounts.
  • Manage the administrative lifecycle of referral source updates via Smartsheet and other internal tracking tools to maintain data integrity.
  • Strictly adhere to all HIPAA regulations and company privacy policies when handling sensitive patient health information.

Qualifications

The ideal candidate is a highly organized professional who thrives in a deadline-driven environment and possesses the interpersonal skills necessary to navigate complex medical office settings.

  • High School Diploma or equivalent required.
  • Proven experience working within an Electronic Medical Record (EMR) system, preferably in a home health or clinical setting.
  • Advanced proficiency in Microsoft Office Suite (Word, Excel, Outlook) and the ability to learn new software systems quickly.
  • Exceptional communication skills with the ability to maintain professionalism while resolving discrepancies with external medical staff.
  • Superior time management skills and the ability to multitask across multiple platforms without compromising data accuracy.
  • Demonstrated ability to remain productive and focused when faced with high workloads and strict regulatory deadlines.
  • Must have reliable transportation for frequent travel to referral partner locations throughout the designated region.
