Crisis Care Manager

About the Role

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual-first and in-home medical, behavioral, and social care, working alongside families and providers to enhance existing care teams.

As a Crisis Team Case Manager, you will work with the families of behaviorally and medically complex children. Key responsibilities include:

• Providing emotional support and care coordination.
• Connecting families to essential resources.
• Supporting healthcare system navigation following hospital discharge.
• Working within a centralized crisis team of therapists and case managers.

Schedule: Shift work (Mon-Fri 8:00am - 04:30pm CST or 10:30am - 07:30pm CST).
