About the Role
Empassion Health is seeking a compassionate and detail-oriented Care Coordinator to join our mission-driven team. As an integral part of our patient advocacy group, you will bridge the gap between healthcare providers and patients, ensuring that every individual receives the personalized care and attention they deserve. In this role, you will leverage your expertise to navigate complex healthcare systems, streamline communication, and improve the overall patient experience within our innovative healthcare model.
Key Responsibilities- Act as the primary point of contact for patients, families, and healthcare providers to ensure seamless care transitions and service delivery.
- Coordinate clinical appointments, follow-up visits, and diagnostic testing while maintaining accurate electronic health records (EHR).
- Monitor patient progress and adherence to care plans, proactively identifying and resolving barriers to care.
- Collaborate with multidisciplinary teams, including physicians and social workers, to optimize health outcomes and resource allocation.
- Educate patients on their treatment plans, insurance benefits, and available community resources.
- Conduct regular telephonic check-ins to assess patient wellness and facilitate timely intervention when necessary.
- Ensure compliance with HIPAA regulations and organizational policies regarding patient data privacy and security.
- Identify opportunities for process improvement within the care delivery workflow to enhance operational efficiency.
- Proven experience in care coordination, case management, or a similar patient-facing administrative role within the healthcare industry.
- Deep understanding of medical terminology, health insurance processes, and clinical workflows.
- Strong interpersonal skills with the ability to build rapport and demonstrate empathy toward patients from diverse backgrounds.
- Excellent organizational skills and the ability to manage multiple priorities in a fast-paced startup environment.
- Proficiency in utilizing Electronic Health Record (EHR) systems and modern digital communication tools.
- High school diploma or equivalent required; a degree in Healthcare Administration, Nursing, or a related field is preferred.
- Relevant certifications such as Certified Case Manager (CCM) or similar credentials are a plus.
- Demonstrated ability to work independently and effectively in a fully remote environment.
